In woodland mansions. Every chest in a mansion has a 50% chance to hold one, so check them all.

Put it in a crafting table with 7 diamonds and cobblestone. You get 2 Vex templates back, so each craft adds one.

4, one for each armor piece. Find one and dupe it 3 times, which costs 21 diamonds and 3 cobblestone.
