In strongholds. Every library chest has one, and the altar chests in the hallways have a 10% chance.

Put it in a crafting table with 7 diamonds and end stone. You get 2 Eye templates back, so each craft adds one.

4, one for each armor piece. Find one and dupe it 3 times, which costs 21 diamonds and 3 end stone.

At a smithing table. Put the Eye template in the first slot, the armor in the second and a trim material like gold or diamond in the third. The template is used up.
