In vaults in trial chambers, with a 6.25% chance per vault. Open them with trial keys from the trial spawners. Each vault rewards you once.

Put it in a crafting table with 7 diamonds and a block of copper. You get 2 Bolt templates back, so each craft adds one. Waxed copper works too, oxidized copper doesn't.

4, one for each armor piece. Find one and dupe it 3 times, which costs 21 diamonds and 3 blocks of copper.

At a smithing table. Put the Bolt template in the first slot, the armor in the second and a trim material like gold or diamond in the third. The template is used up.
