Y -2. Lapis spawns from the bottom of the world up to Y 64 and is most common right around Y 0, where stone turns into deepslate.
About two thirds of all lapis comes from veins that never touch air, so caves only show you a small part of it. Dig tunnels at Y -2 instead.
A stone pickaxe or better. A wooden or gold pickaxe breaks lapis ore without dropping anything.
4 to 9 lapis lazuli. Fortune multiplies that, so Fortune III can drop up to 36.
